If the words in Spinoza’s Ethics are considered as symbols, then certain words in the definitions of the Ethics can be replaced with symbols from set theory and we can reexamine Spinoza’s first definitions within a logical–mathematical frame. The authors believe that, some aspects of Spinoza’s work can be explained and illustrated through mathematics. A semantic relation between the definitions of the philosopher and set theory is presented. It is explained each chosen symbol. The ontological argument is developed through modal (...) logic. Aim: Patients with advanced cancer need information about end-of-life treatment options in order to make informed decisions. Clinicians vary in the frequency with which they initiate these discussions.Patients and methods: As part of a long-term longitudinal study, patients with an expected 2-year survival of less than 50% who had advanced gastrointestinal or lung cancer or amyotrophic lateral sclerosis were interviewed. Each patient’s medical record was reviewed at enrollment and at 3 months for evidence of the discussion of patient wishes concerning (...) ventilator support, artificial nutrition and hydration , resuscitation and hospice care. A Kaplan–Meier analysis was also performed and 2-year survival calculated.Results: 60 cancer and 32 ALS patients were enrolled. ALS patients were more likely than cancer patients to have evidence of discussion about their wishes for ventilator support , ANH , DNR and hospice care . At 6 months, 91% of ALS patients were alive compared with 62% of cancer patients; at 2 years, 63% of ALS patients were alive compared with 23% of cancer patients .Conclusions: Cancer patients were less likely than ALS patients to have had documented advanced care planning discussions despite worse survival. This may reflect perceptions that ALS has a more predictable course, that advanced cancer has a greater number of treatment options, or differing views about hope. Nevertheless, cancer patients may be less adequately prepared for end-of-life decision-making. (shrink)

This collection of papers by prominent feminist thinkers advances the positive feminist project of remapping the moral by developing theory that acknowledges the diversity of women.

This paper argues that professionals who make boundary-related decisions should be guided by relationship-based practice. In our roles as service users and professionals, drawing from our lived experiences of professional relationships, we argue we need to move away from distance-based practice. This includes understanding the boundary stories and narratives that exist for all of us – including the people we support, other professionals, as well as the organisations and systems within which we work. When we are dealing with professional boundary (...) issues, we should centre relationship-building skills that are central to many other aspects of our work. Skills that foster relationships at all levels – between professionals, service users, and services – need to be revalued. Our final recommendation is to create, develop, and foster safer spaces within and outside of organisations, as well as inter-professionally, for the discussion and exploration of boundary-related issues and practice. We are interested in hearing from those with experiences of being marginalised by boundaries so that they can inform a reshaping of our collective ideas around boundary related practices. To foster relationship-based practices in organisations, we have outlined several recommendations here; however, we recognize that these do not go far enough, and that collective action is needed to inform systemic change. Extensive research has demonstrated that rs1360780, a common single nucleotide polymorphism within the FKBP5 gene, interacts with early-life stress in predicting psychopathology. Previous results suggest that carriers of the TT genotype of rs1360780 who were exposed to child abuse show differences in structure and functional activation of emotion-processing brain areas belonging to the salience network. Extending these findings on intermediate phenotypes of psychopathology, we examined if the interaction between rs1360780 and child abuse predicts resting-state functional connectivity (rsFC) between the amygdala (...) and other areas of the salience network. We analyzed data of young European adults from the general population (N = 774; mean age = 18.76 years) who took part in the IMAGEN study. In the absence of main effects of genotype and abuse, a significant interaction effect was observed for rsFC between the right centromedial amygdala and right posterior insula (p <.025, FWE-corrected), which was driven by stronger rsFC in TT allele carriers with a history of abuse. Our results suggest that the TT genotype of rs1360780 may render individuals with a history of abuse more vulnerable to functional changes in communication between brain areas processing emotions and bodily sensations, which could underlie or increase the risk for psychopathology. BackgroundStructured, systematic methods to formulate consensus recommendations, such as the Delphi process or nominal group technique, among others, provide the opportunity to harness the knowledge of experts to support clinical decision making in areas of uncertainty. They are widely used in biomedical research, in particular where disease characteristics or resource limitations mean that high-quality evidence generation is difficult. However, poor reporting of methods used to reach a consensus – for example, not clearly explaining the definition of consensus, or not stating (...) how consensus group panellists were selected – can potentially undermine confidence in this type of research and hinder reproducibility. Our objective is therefore to systematically develop a reporting guideline to help the biomedical research and clinical practice community describe the methods or techniques used to reach consensus in a complete, transparent, and consistent manner.MethodsThe ACCORD project will take place in five stages and follow the EQUATOR Network guidance for the development of reporting guidelines. In Stage 1, a multidisciplinary Steering Committee has been established to lead and coordinate the guideline development process. In Stage 2, a systematic literature review will identify evidence on the quality of the reporting of consensus methodology, to obtain potential items for a reporting checklist. In Stage 3, Delphi methodology will be used to reach consensus regarding the checklist items, first among the Steering Committee, and then among a broader Delphi panel comprising participants with a range of expertise, including patient representatives. In Stage 4, the reporting guideline will be finalised in a consensus meeting, along with the production of an Explanation and Elaboration document. In Stage 5, we plan to publish the reporting guideline and E&E document in open-access journals, supported by presentations at appropriate events. Dissemination of the reporting guideline, including a website linked to social media channels, is crucial for the document to be implemented in practice.DiscussionThe ACCORD reporting guideline will provide a set of minimum items that should be reported about methods used to achieve consensus, including approaches ranging from simple unstructured opinion gatherings to highly structured processes. J’y ai soutenu que, pour des raisons de justice, l’État devrait distribuer une partie du vaccin rare qu’il pourrait détenir à certains de ses employés de la santé et le reste aux citoyens de manière aléatoire et égale sur le principe (...) d’une loterie. Au coeur de la critique de Saunders se trouve l’affirmation selon laquelle j’ai omis à tort de prendre en compte les différences moralement pertinentes dans les besoins et la capacité de production entre les receveurs potentiels du vaccin. Le point central de ma réponse ici est que le fait que le besoin et la capacité de production soient ou non des facteurs moralement pertinents dépend de qui ou de quoi distribue le vaccin, à qui ils le distribuent et pourquoi ils le font. Par exemple, la discrimination entre les personnes dans la distribution de vaccins publics distribués à titre prophylactique sur la base de leur âge est injuste, qu’elle soit ou non moralement justifiable pour d’autres motifs. Il y eut plus de 1 900 contributeurs à la section mathématique du journal Educational Times durant ses 67 ans d’existence. Cette rubrique mensuelle contenait des problèmes et leurs solutions, de courts articles, et souvent de courts comptes rendus de l’assemblée la plus récente de la London Mathematical Society. Hugh MacColl (1837-1909), enseignant et tuteur, était l’un des contributeurs les plus prolifiques à cette section. Il soumettait ses contributions depuis Boulogne-sur-Mer, France. Son expertise portait essentiellement sur la probabilité géométrique, les (...) probabilités classiques et la logique. Après un bref aperçu de l’histoire de la section mathématique du Educational Times et de la correspondance de MacColl avec son directeur de publication, W. J. C. Our goal here is to help identify the contextualist’s most worthy competitor to relativism. Recently, some philosophers of language and linguists have argued that, while there are contextualist-friendly semantic theories of deontic modals that fit with the relativist’s challenge data, the best such theories are not Lewis-Kratzer-style semantic theories. If correct, this would be important: It would show that the theory that has for many years enjoyed the status of the default view of modals in English and other languages is (...) in need of revision. Here we defend the default view by showing how a Kratzer-style semantics is able to make available readings of the relevant utterances that fit with the pretheoretical judgments opponents purport it cannot fully capture. Having established this, we turn to considering the more theoretical grounds proponents have offered for preferring their rival contextualist views. Here the question is to what extent such grounds favor semantic over what Korta and Perry call “near-side pragmatic” explanations of our judgments. In particular, we argue that our favored readings figure in near-side pragmatic explanations of those judgments that possess the methodological and theoretical advantages of systematicity and unity at least as well as, if not to a greater extent than, those of opponents who argue for their revised semantic theories on the basis of these advantages. Aristotle is widely regarded as the greatest of all philosophers; indeed, he is traditionally referred to simply as `the philosopher'. Today, after more than two millennia, his arguments and ideas continue to stimulate philosophers and provoke them to controversy. In this book J.L. Ackrill conveys the force and excitement of Aristotle's philosophical investigations, thereby showing why contemporary philosophers still draw from him and return to him. He quotes extensively from Aristotle's works in his own notably clear English translation, and a (...) picture emerges of a lucid, lively, subtle and tough-minded thinker of astonishing range and penetration. Professor Ackrill identifies many striking connections between Aristotle's ideas and ideas in recent philosophy; he also raises philosophical questions of his own, and exemplifies the way in which Aristotle can still be argued with and learned from. Throughout the history of philosophy there has been a sustained interest in the concepts of knowledge, truth and meaning; interest in the concepts of error, falsity and nonsense, on the other hand, has been intermittent and spasmodic. Error, for example, has suffered at the expense of knowledge to such an extent that sometimes its very existence has been denied, or it has been explained away as being merely the absence of or privation of knowledge; many theories of truth are so (...) constructed that no place can be found for falsity, and theories about what constitutes making sense pay, on the whole, little heed to what constitutes nonsense. In this paper I hope to do something to redress the balance so far as error is concerned. My remarks are prompted by the hope that, just as we may best understand health through the study of disease, so a consideration of error or failure may throw light on knowledge or success. It is clearly not very informative to say of error, falsity and nonsense that they are merely the absence of knowledge, truth and sense; indeed it is just as laconic as a proposed medical definition of disease as the absence of health.
